The below text is hereby appended to the INB Preauthorized Transfer Request and Automatic ACH Transfer Authorization:

This Preauthorized Transfer Request and Automatic ACH Transfer Authorization will remain in full force and effect until I/we notify INB in writing at 322 E. Capitol Ave., Springfield, IL 62701 or by phone at 217-747-5500 or by email at customerservice@inb.com  that I/we wish to revoke this authorization. I/we understand that INB requires at least 5 days prior notice in order to cancel this authorization.
